Case Management Leadership - A Collaborative Force
This presentation will focus on todays acute healthcare system environment and the role case management plays as a leader within the system. The participants will be able to relate to current challenges and issues facing case management departments' within primarily acute healthcare. Case Management is a" professional practice" ......versus a "job" and what that means in regards to everyone's role as a leader within the practice.
- Identify the leadership role cm plays in the development and d implementation of the transition of care plan.
- Understand the value of varied approaches to communication with all team members to reach common goals impacting all stakeholders.
- Identify management strategies and tools designed to improve the role and value of CM
